New OCI Card-Adult – US PASSPORT HOLDER (INDIVIDUAL)
- The documents in the Checklist are mandatory for your application to be processed.
- This list of documents is not exhaustive and is subject to change based on the requirements of the
Indian Embassy / Consulate.
- The Indian Embassy / Consulate have the right to request for additional documentation.
- 2 copies of signed Checklist with the boxes ticked must be submitted with your documents.
OCI APPLICATION FORM
- Complete the OCI Online Application Form on the Government Site
- Once the OCI Online Application Form is completed and printed, the following should be submitted along with the application package
- Step 1 :-
- One Part A
- AND
- Step 2 :-
- One Part B
- Step 1 :-
- Print out PART A and PART B.
- Submit BOTH parts for the OCI Application.
SIGNATURE
- OCI form has multiple places for signatures. The following sections must be SIGNED correctly.
- OCI PART A – EACH Family member MUST have a separate FORM A duly signed by that family member separately.
- OCI PART B – COMMON for all family members, EACH APPLICANT MUST SIGN the same FORM B.
NOTE -If the signature is OUTSIDE of the boxes provided, the application will be REJECTED.
CKGS Disclaimer Form
- This must have Signature of the applicant.
ADDITIONAL PARTICULARS FORM
- Fill the Additional Particular Form and enclose the print out.
- All pages of additional particulars form should have signatures of the applicant
- All questions must be answered – no question should be left blank
PHOTOS
- 2 most recent passport size photo(no older than 6 months) of the applicant to be provided
- Photos to be 2 inches x 2 inches in size, in color, depicting front pose strictly against white background only on photographic paper.
COPY OF CURRENT US PASSPORT
- Self-Attested Color Copy of the following pages of valid US Passport
- Information page that contains the picture and basic details of the US passport.
- Endorsement / amendment pages.
- Passport should be valid for 8 months from the date of application.
NOTE: DO NOT SUBMIT ORIGINAL US PASSPORT AT THE TIME OF SENDING INITIAL OCI
APPLICATION. ORIGINAL US PASSPORT REQUIRED ONLY AT THE TIME OF OCI MATCHUP.
IMPORTANT: Applications submitted without the above will be considered INCOMPLETE
and processing will be delayed.
BIRTH CERTIFICATE
- For applicants born outside of India, a photocopy of the birth certificate OR any government issued document that shows the following must be provided:-
- Parents Name
- Date of Birth
- Nationality
- If the applicant is applying through parents, one parent was born outside India then Birth Certificate and Passport copy of the parent born outside India.
- If the BIRTH CERTIFICATE / Documents are in any other language besides English, the document needs to be translated into English and it has to be notarized.
NATURALIZATION CERTIFICATE
- If you acquired US Nationality through Naturalization, a copy of your Naturalization Certificate is required.
- If you do not have an Indian passport and is applying for OCI on the basis that the parents and/or grandparents were Indian nationals – then PARENTS AND/OR GRANDPARENTS NATURALIZATION CERTIFICATE IS NEEDED
- If the applicant has more than one NATIONALITY after having INDIAN Citizenship, please provide all NATURALIZATION CERTIFICATES or PROOF OF NATIONALITY from all countries where the applicant has held citizenship.
- The US naturalization Certificate should state ‘FORMER NATIONALITY’ AS INDIA. Certificates that state ‘Country of Birth’ as India CANNOT be accepted. If gained naturalization through his/her parents and possesses no separate naturalization certificate, then copies of the parents naturalization certificates and a Notarized statement signed by BOTH parents is required stating the circumstances that their child received his/her naturalization status and why the child does not possess his/her own certificate.
PROOF OF INDIAN ORIGIN
- Copy of first three and last two pages of the last Indian passport held
OR
- Any document that provides sufficient evidence that the applicant has held
- Indian Citizenship like:-
- Any other proof substantiating the request. Usually applicants are able to submit a certificate of residence or place of birth of self / parents / grandparents from First Class Magistrate / District Magistrate (DM) of the concerned place.
- Notarized copy of educational certificate issued by an institution of India and land ownership certificate by which eligibility may be reasonably ascertained
- OR
- A duly notarized affidavit to that effect
- OR
- A copy of an Ancestral Nativity Certificate issued by an Indian official or magistrate with sufficient authority to confirm Indian Origin of the applicant.
- Indian Citizenship like:-
- The decision to whether accept this type of document as proof of Indian Citizenship will be made by the Embassy/Consular Officer and will be FINAL in such cases
PARENTS/GRANDPARENTS PROOF OF ORIGIN
- If the applicant does NOT HAVE an Indian Passport and is applying for OCI on the basis that his parents and / or grandparents were Indian Nationals, then the applicant must enclose the following documents:-
- Copy of Proof of both parents current nationality and previous nationality
- Applicants birth certificate (in which your parent’s name is mentioned)
- If the applicant is applying as a grandchild of an Indian National, then the applicant must submit his birth certificate as well as his parent’s birth certificate. All submitted birth certificates must show the parent’s names clearly so that it establishes the relationship with the applicants parents and grandparents
- AND
- Copies of first three and last two pages of BOTH parent/grandparent’s Indian passport,
- OR
- Copy of BOTH parent/grandparent’s domicile certificate issued by the competent authority,
- OR
- Copy of parents/grandparent’s OCI cards including BOTH information pages on OCI cards
Note: If parents / grandparents have OCI / PIO cards, it is mandatory to give copies of BOTH – Indian Passport & OCI Card.
PARENTS PROOF OF RENUNCIATION IF THE APPLICANT NEVER HELD INDIAN PASSPORT AND IS APPLYING FOR OCI ON THE BASIS OF PARENTS:
- If one parent or both parents are naturalized citizens, proof of renunciation for parent(s), must be provided:-
- Certificate of renunciation issued by an Embassy/Consulate of India
- OR
- Cancelled Indian Passport with stamp stating the passport is cancelled due to obtaining Foreign Citizenship (Non US)
If parent NEED to obtain RENUNCIATION CERTIFICATE, then they need to apply for renunciation certificate prior to submitting application for Adults.
PROOF OF ADDRESS
Proof of address to match with present address as per application. Only one of the following:
- Only State Issued – Driving Licence (validity for at least 6 months pending)
OR
- Utility Bill – Water (should be recent, no more than 3 months old)
OR
- Utility Bill – Electricity (should be recent, no more than 3 months old)
OR
- Utility Bill – Gas (should be recent, no more than 3 months old)
OR
- Valid lease signed by landlord and tenant (applicable ONLY FOR Atlanta and Houston. For New York, lease document must be notarized.)
- For College / University Students living on campus in University housing/dormitories, a letter from your institution on the institution letter head explaining the exact address that you presently reside in will suffice (Not Applicable for Applications from Washington DC).
NOTE: DO NOT CUT YOUR DRIVER’S LICENCE COPY DOWN TO A SMALLER SIZE
NAME CHANGE DOCUMENTATION
- If your name in any of the supporting documents provided is different in any way from the name in the US Passport, a copy of ONE of the following documents must be provided
- Marriage Certificate showing the name change if the name was changed at marriage
- Official Court Order for the name change
- Endorsement of name change during naturalization process
- If in case the middle name was changed to an initial then a notarized statement letter is required.
PROOF OF RENUNCIATION OF INDIAN CITIZENSHIP:
- If you ever held an Indian Passport, it is MANDATORY to provide proof of Renunciation of Indian Citizenship along with your OCI application. One of the following MUST be produced as proof:-
- Certificate of renunciation issued by the Embassy/Consulate of India
- Cancelled Indian Passport with stamp stating the passport is cancelled due to obtaining US citizenship
- Deemed surrender certificate issued by the Embassy/Consulate of India
